Uses

Hyzaar is used to treat hypertension, and to reduce the risk of stroke in patients with left ventricular hypertrophy and hypertension. 

Dosage

Precautions

There are two active ingredients in Prempro: conjugated estrogen and medroxyprogesterone acetate.

What is Prempro used for?

Prempro is prescribed for women suffering from menopause related symptoms. As long they have their uterus still, Prempro medication is going to work. It contains progestin and estrogen hormones, which are naturally produced by the female body. Appearing in the form of a tablet, Prempro or conjugated estrogens/medroxyprogesterone acetate is taken just once per day.

It is used to treat serious hot flashes caused by menopause. Additionally, Prempro is indicated for moderate to severe burning, itching, or dryness inside the vagina or around it. Finally, it is used to prevent bone loss or osteoporosis in menopausal women. If you haven’t had a hysterectomy, this medicine can help you.

 Here is how it works. When undergoing menopause and afterwards, the female body stops  producing high levels of estrogen and the levels that are produced tend to be imbalanced. Additionally, the body starts producing a type of estrogen called androstenedione instead of estradiol(produced during a woman’s reproductive age). Androstenedione is released by adrenal glands rather than ovaries and then turned into estrone estrogen. When there is a low level of estradiol and a high level of estrone, menopausal symptoms result.

As Prempro contains equine estrogens, totally natural, plus a progestin hormone called medroxyprogesterone, it can easily relieve symptoms of menopause. It will provide the estradiol estrogen that the ovaries stop producing. This type of estrogen will strengthen bones and prevent osteoporosis. It has a lower risk of uterine cancer because of the medroxyprogesterone.

How is the dosage best taken?

If taking Prempro, it will be necessary to take it correctly to benefit from it. Prempro is in a tablet form and taken once per day. It is fine to take it with or without a meal. If after taking it for the first time you experience stomach upset, it will be best to take it with food henceforth. Taking Prempro at any time of day is fine, but it is good to have a strict schedule to maintain even levels of the drug in your blood.

Uses

Prevacid (lansoprazole) belongs to a class of medications known as proton pump inhibitors (PPIs). PPIs work by reducing the amount of acid in your stomach.

Excess stomach acid can cause a burning sensation known as heartburn. Over time, excess stomach acid can damage the stomach lining or esophagus. Medications that reduce stomach acid can heal this damage and relieve heartburn symptoms.

Prevacid is prescribed for use in adults, children, and infants for many conditions related to stomach acid. A few examples include:

  • Acid reflux or GERD (gastroesophageal reflux disease)
  • Heal or prevent ulcers of the stomach or esophagus
  • H. Pylori stomach infection, along with antibiotics

Dosage

The over-the-counter (OTC) Prevacid 24HR contains the same active ingredient as prescription (Rx) Prevacid. Prevident 5000 Plus (Sodium Fluoride) Dosage and Side Effects

Sodium fluoride makes teeth more resistant to decay and bacteria that cause cavities. This medication is used to prevent cavities.

Proper Use of this medication

This is best used once daily just before bedtime instead of your regular toothpaste or as directed by your healthcare professional.

Apply a thin ribbon of medicine to a toothbrush and brush thoroughly for 2 minutes or as directed.

Spit out the medication after use. Do not swallow. Children should rinse their mouth thoroughly right away. For adults, do not rinse your mouth, eat, or drink for at least 30 minutes after use to get the best results.

Side Effects

Mouth/gum irritation may rarely occur. If this effect persists or worsens, tell your doctor/dentist or pharmacist promptly.

Tell your doctor/dentist right away if your teeth become stained or pitted. This is often a result of too much fluoride.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Warnings and Precautions

Before using sodium fluoride, tell your healthcare professional if you are allergic to it; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.

Before using this medication, tell your doctor/dentist or pharmacist your medical history, especially of: mouth problems (e.g., sores, mucositis).

Children may be more sensitive to the side effects of this drug, especially if this medication is swallowed. Watch children closely when they are using this medication to make sure that they do not swallow this medication.

Before using this medicine, tell your healthcare professional if you are pregnant.

It is not known if this drug passes into breast milk. Consult your healthcare professional before breastfeeding.

PREVPAC HP PAC is used in people with Helicobacter pylori (H. pylori) infection and stomach ulcers. Treating H. pylori infection can help prevent future stomach ulcers.

Proper Use of this medication

Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended.

Each dose of this combination medicine contains 4 pills (3 capsules and 1 tablet). Take all 4 pills twice daily before you eat, unless your doctor tells you otherwise.

Do not crush, chew, break, or open the pills. Swallow each pill whole. You may swallow the pills one at a time.

Use this medicine for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antibiotics. This medicine will not treat a viral infection such as the flu or a common cold.

This medicine can cause unusual results with certain lab tests for glucose (sugar) in the urine. Tell any doctor who treats you that you are using PREVPAC-HP PAC.

Store at room temperature, away from moisture, heat, and light.

Seek emergency medical attention or call the Poison Help line.

Take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

Side Effects

Get emergency medical help if you have signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have:

  • diarrhea that is watery or bloody;
  • headache with chest pain and severe dizziness, fainting, fast or pounding heartbeats;
  • liver problems--nausea, upper stomach pain, itching, tired feeling, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes);
  • kidney problems--urinating more or less than usual, blood in your urine, swelling, rapid weight gain;
  • signs of inflammation in your body--swollen glands, flu symptoms, easy bruising or bleeding, severe tingling or numbness, muscle weakness, chest pain, new or worsening cough with fever, trouble breathing; or
  • severe skin reaction--fever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain, followed by a red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling.

Serious side effects may be more likely in older adults.

Common side effects may include:

  • diarrhea, constipation;
  • headache;
  • vaginal itching or discharge;
  • nausea, vomiting, stomach pain;
  • unusual or unpleasant taste in the mouth; or
  • discolored teeth, black or "hairy" tongue.

Warnings and Precautions

You should not take this medicine if you have a history of long QT syndrome or certain heart rhythm disorders, or a history of liver problems caused by taking clarithromycin.

You also should not take this medicine if you have ever had a severe allergic reaction to certain antibiotics, including azithromycin (Zithromax, Z-Pak), erythromycin, telithromycin, amoxicillin or other penicillins (ampicillin, Amoxil, Augmentin, Trimox, and others), or cephalosporins (Duricef, Keflex, Spectracef, and others).

Serious drug interactions can occur when certain medicines are used together. Tell each of your healthcare providers about all medicines you use now, and any medicine you start or stop using.

You should not take this medicine if you are allergic to amoxicillin (Amoxil), clarithromycin (Biaxin), or lansoprazole (Prevacid), or if:

  • you have a history of long QT syndrome or certain heart rhythm disorders;
  • you have ever had liver problems or jaundice caused by taking clarithromycin;
  • you are allergic to azithromycin (Zithromax, Z-Pak), erythromycin, or telithromycin;
  • you have had a severe allergic reaction to a penicillin antibiotic, including ampicillin, Augmentin, Principen, Timentin, Trimox, and others; or
  • you have had a severe allergic reaction to a cephalosporin antibiotic such as cefdinir, cefprozil, cefuroxime, cephalexin, Duricef, Omnicef, Cefzil, Keflex, Spectracef, and others.

Some medicines can cause unwanted or dangerous effects when used with PREVPAC-HP PAC. Your doctor may need to change your treatment plan if you use any of the following drugs:

  • cisapride;
  • colchicine (if you also have liver or kidney disease);
  • pimozide;
  • lovastatin (Advicor, Altoprev, Mevacor) or simvastatin (Zocor, Simcor, Vytorin, Juvisync); or
  • ergot medicine--dihydroergotamine, ergotamine, ergonovine, methylergonovine.

To make sure PREVPAC-HP PAC is safe for you, tell your doctor if you have:

  • liver disease;
  • kidney disease;
  • allergies;
  • myasthenia gravis;
  • osteoporosis or low bone mineral density (osteopenia); or
  • an electrolyte imbalance or metabolic disorder.

Taking a proton pump inhibitor such as lansoprazole may increase your risk of bone fracture in the hip, wrist, or spine. This effect has occurred mostly in people who have taken the medication long term or at high doses, and in those who are age 50 and older. It is not clear whether lansoprazole is the actual cause of an increased risk of fracture.

It is not known whether this medicine will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant.

Amoxicillin can make birth control pills less effective. Ask your doctor about using non hormonal birth control (condom, diaphragm with spermicide) to prevent pregnancy while taking PREVPAC-HP PAC.

Amoxicillin and lansoprazole can pass into breast milk and may harm a nursing baby. You should not breast-feed while using this medicine.

This medicine is not approved for use by anyone younger than 18 years old.

Interactions with this medication

This medicine can cause diarrhea, which may be a sign of a new infection. If you have diarrhea that is watery or bloody, call your doctor. Do not use anti-diarrhea medicine unless your doctor tells you to.

If you also take sucralfate (Carafate), avoid taking it within 30 minutes after you take PREVPAC-HP PAC. Sucralfate can make it harder for your body to absorb lansoprazole.

A gout medicine called colchicine (Colcrys) can interact with clarithromycin and cause serious or life-threatening medical problems. You should not take colchicine during your treatment with PREVPAC-HP PAC, especially if you are an older adult with kidney disease.

Many other drugs can interact with clarithromycin. Not all possible interactions are listed here. Tell your doctor about all your medications and any you start or stop using during treatment with PREVPAC-HP PAC, especially:

  • atorvastatin (Lipitor, Caduet) or pravastatin (Pravachol);
  • carbamazepine;
  • insulin or oral diabetes medicine (especially nateglinide, pioglitazone, repaglinide, or rosiglitazone);
  • mycophenolate mofetil;
  • warfarin (Coumadin, Jantoven);
  • HIV or AIDS medicine--atazanavir, ritonavir, saquinavir;
  • a Valium-type sedative--alprazolam, midazolam, triazolam;
  • heart or blood pressure medication--amlodipine, diltiazem, verapamil;
  • heart rhythm medication--amiodarone, disopyramide, dofetilide, dronedarone, ibutilide, procainamide, quinidine, sotalol; or
  • sildenafil (Viagra) and other erectile dysfunction medicines--avanafil (Stendra), tadalafil (Cialis), vardenafil (Levitra).

This list is not complete and many other drugs can interact with PREVPAC-HP PAC. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Give a list of all your medicines to any healthcare provider who treats you.

Prezcobix (Darunavir/Cobicistat) Dosage and Side Effects

PREZCOBIX is used for the treatment of HIV (Human Immunodeficiency Virus) infection in adults when co-administered with other antiretroviral medications.

Proper Use of this medication

Always use PREZCOBIX exactly as your doctor has told you. You must check with your doctor if you are not sure.

You should always take PREZCOBIX with food. The type of food is not important.

Continue taking PREZCOBIX unless your doctor tells you to stop. Take the exact amount of PREZCOBIX that your doctor tells you to take, right from the very start. To help make sure you will benefit from PREZCOBIX, you must not skip doses or interrupt therapy. If you don’t take PREZCOBIX as prescribed, the beneficial effects of PREZCOBIX may be reduced or even lost.

If you have also been prescribed enteric-coated didanosine as well as PREZCOBIX, take didanosine 2 hours before or after PREZCOBIX.

Overdose:

In case of drug overdose, contact a health care practitioner, hospital emergency department or regional Poison Control Centre immediately, even if there are no symptoms.

Missed dose:

If you miss a dose of PREZCOBIX by more than 12 hours, wait and then take the next dose of PREZCOBIX at the regularly scheduled time. If you miss a dose of by less than 12 hours, take your missed dose of PREZCOBIX immediately. Then take your next dose of PREZCOBIX at the regularly scheduled time.

If a dose of PREZCOBIX is skipped, do not double the next dose. Do not take more or less than your prescribed dose of PREZCOBIX at any one time.

Do not stop using PREZCOBIX without talking to your doctor first.

Side Effects

Like all prescription drugs, PREZCOBIX can cause side effects. The following is not a complete list of side effects reported with PREZCOBIX when taken either alone or with other anti-HIV medicines. Do not rely on this leaflet alone for information about side effects. Your doctor can discuss with you a more complete list of side effects.

Liver problems that may occasionally be severe have been reported. Your doctor should do blood tests prior to initiating PREZCOBIX. If you have chronic hepatitis B or C infection, your doctor should check your blood tests more often because you have an increased chance of developing liver problems.

Talk to your doctor about the signs and symptoms of liver problems. These may include yellowing of your skin or whites of your eyes, dark (tea coloured) urine, pale coloured stools (bowel movements), nausea, vomiting, loss of appetite, or pain, aching, or sensitivity on your right side below your ribs.

Rash has been reported in 15.7% of patients receiving PREZCOBIX. In patients taking PREZCOBIX and raltegravir, rashes (generally mild or moderate) may occur more frequently than in patients taking either drug separately. Contact your doctor immediately if you develop a rash. Your doctor will advise you whether your symptoms can be managed on therapy or whether PREZCOBIX should be stopped.

In some patients, severe or life-threatening rash has been reported. If you develop a severe rash (e.g., blisters, peeling skin) which may be accompanied with symptoms such as fever, fatigue, swelling of the face or lymph glands, muscle aches and pain, and liver problems, immediately discontinue PREZCOBIX and contact your doctor.

Other relevant severe side effects reported at an uncommon or rare frequency were inflammation of the liver or pancreas, increased blood fat levels, diabetes, and changes in body fat.

The most common side effects include diarrhea, nausea, headache, abdominal pain and vomiting.

Some side effects are typical for anti-HIV medicines in the same family as PREZCOBIX. These are:

  • high blood sugar (hyperglycemia) and diabetes. This can happen in patients taking PREZCOBIX or other protease inhibitor medicines. Some patients have diabetes before starting treatment with PREZCOBIX, which gets worse. Some patients get diabetes during treatment with PREZCOBIX. Some patients will need changes in their diabetes medicine. Some patients may need new diabetes medicine.
  • increased bleeding in patients with hemophilia. This may happen in patients taking PREZCOBIX as it has been reported with other protease inhibitor medicines.
  • changes in body fat. These changes can happen in patients taking anti-HIV medicines. The changes may include an increased amount of fat in the upper back and neck, breast, and around the back, chest, and stomach area. Loss of fat from the legs, arms, and face may also happen. The exact cause and long-term health effects of these conditions are not known.
  • increases in triglycerides and cholesterol (forms of fat that are found in your blood). Your doctor may order blood testing for you.
  • development of pancreatitis (inflammation of the pancreas) with symptoms such as abdominal pain, nausea and vomiting. If you suffer these symptoms while taking PREZCOBIX, contact your doctor.
  • Changes in your immune system (Immune Reconstitution Inflammatory Syndrome) can happen when you start taking HIV medicines. Your immune system may get stronger and begin to fight infections that have been hidden in your body for a long time, or you could develop an autoimmune disease in which your immune system reacts against your own body (e.g. Graves' disease (which affects the thyroid gland), Guillain-Barré syndrome (which affects the nervous system) or polymyositis (which affects the muscles) and it may develop at any time, sometimes months later after the start of HIV therapy). Sometimes symptoms can be severe, so if you develop high temperature (fever), joint or muscle pain, redness, rash, swelling, or fatigue or any new symptoms contact your doctor straight away.

Tell your doctor promptly about these or any other unusual symptoms. If the condition persists or worsens, seek medical attention.

Warnings and Precautions

BEFORE you use PREZCOBIX talk to your doctor or pharmacist if you:

  • have diabetes. In general, anti-HIV medicines, such as PREZCOBIX, might increase sugar levels in the blood.
  • have liver problems, including hepatitis B and/or C.
  • have hemophilia. Anti-HIV medicines, such as PREZCOBIX, might increase the risk of bleeding.
  • are pregnant or planning to become pregnant. It is not known if PREZCOBIX can harm your unborn baby. You and your doctor will need to decide if takingPREZCOBIX is right for you. If you take PREZCOBIX while you are pregnant, talk to your doctor about how you can be included in the Antiretroviral Pregnancy Registry.
  • are breast-feeding. Do not breast-feed if you are taking PREZCOBIX. You should not breast-feed if you have HIV because of the chance of passing HIV to your baby. Talk with your doctor about the best way to feed your baby.
  • are allergic to sulpha medications.
  • are less than 18 years of age.

Interactions with this medication

Tell your doctor about all the medicines you take including prescription and nonprescription medicines, vitamins, and herbal supplements, including St. John’s wort (Hypericum perforatum). PREZCOBIX and many other medicines can interact. Sometimes serious side effects will happen if PREZCOBIX is taken with certain other medicines.

PREZCOBIX should not be combined with vardenafil, because you may be at increased risk of side effects of vardenafil such as low blood pressure, visual changes and penile erection lasting more than 4 hours.

Tell your doctor if you are taking estrogen-based contraceptives. PREZCOBIX might reduce the effectiveness of estrogen-based contraceptives (birth control). Therefore, alternative methods of non-hormonal contraception, such as a condom, are recommended.

Tell your doctor if you take other anti-HIV medicines (e.g. rilpivirine, tenofovir disoproxil fumarate). PREZCOBIX can be combined with some other anti-HIV medicines while other combinations are not recommended.

If you are taking PREZCOBIX you should not take:

  • medications that may affect your kidneys and have not been discussed with your doctor
  • other medicines that contain protease inhibitors: e.g., atazanavir (Reyataz), indinavir (Crixivan), saquinavir (Invirase), lopinavir (Kaletra), or darunavir (Prezista)
  • other medicines that contain cobicistat (Stribild)
  • Tell your doctor if you are taking any medicines that you obtained without a prescription.
  • This is not complete list of medicines that you should tell your doctor that you are taking. Know and keep track of all the medicines you take and have a list of them with you. Show this list to all of your doctors and pharmacists any time you get a new medicine. Both your doctor and your pharmacist can tell you if you can take these other medicines with PREZCOBIX. Do not start any new medicines while you are taking PREZCOBIX without first talking with your doctor or pharmacist. You can ask your doctor or pharmacist for a list of medicines that can interact with PREZCOBIX.

Humulin R Dosage and Side Effects

HUMULIN  R is used to treat diabetes.

Proper Use of this medication

Your blood sugar will need to be checked often, and you may need other blood tests at your doctor's office.

Insulin is injected under the skin. You will be shown how to use injections at home. Use a different place on your stomach, thigh, or upper arm each time you give the injection. Do not inject into the same place two times in a row.

Shake the insulin vial (bottle) several times to thoroughly mix the HUMULIN 70/30 before each use. Shake the mixture until it looks cloudy or milky.

Do not use the mixture if it has clumps or white particles in it after mixing, or if the white substance remains at the bottom of the vial. Call your doctor for a new prescription.

Use a disposable needle only once, then throw away in a puncture-proof container (ask your pharmacist where you can get one and how to dispose of it). Keep this container out of the reach of children and pets.

Never share an injection pen or cartridge with another person. Sharing injection pens or cartridges can allow disease such as hepatitis or HIV to pass from one person to another.

Low blood sugar (hypoglycemia) can happen to everyone who has diabetes. Symptoms include headache, hunger, sweating, pale skin, irritability, dizziness, feeling shaky, or trouble concentrating. Always keep a source of sugar with you in case you have low blood sugar. Sugar sources include fruit juice, hard candy, crackers, raisins, and non-diet soda. Be sure your family and close friends know how to help you in an emergency.

If you have severe hypoglycemia and cannot eat or drink, use a glucagon injection. Your doctor can prescribe a glucagon emergency injection kit and tell you how to use it.

Also watch for signs of high blood sugar (hyperglycemia) such as increased thirst, increased urination, hunger, dry mouth, fruity breath odor, drowsiness, dry skin, blurred vision, and weight loss.

Check your blood sugar carefully during times of stress, travel, illness, surgery or medical emergency, vigorous exercise, or if you drink alcohol or skip meals. These things can affect your glucose levels and your dose needs may also change. Do not change your medication dose or schedule without your doctor's advice.

Carry an ID card or wear a medical alert bracelet stating that you have diabetes, in case of emergency. Any doctor, dentist, or emergency medical care provider who treats you should know that you are diabetic.

Storing unopened vials or injection pens: Keep in the carton and store in a refrigerator, protected from light. Throw away any insulin not used before the expiration date on the medicine label.

Storing after your first use: Keep "in-use" vials, injection pens, or prefilled syringes at cool room temperature, protected from heat and sunlight. In-use insulin is only stable for a certain number of days or weeks. Throw away the medicine after the number of days or weeks shown in the storage directions provided with your insulin.

Do not freeze HUMULIN 70/30, and throw away the medication if it has become frozen.

Seek emergency medical attention if you think you have used too much of this medicine. An insulin overdose can cause life-threatening hypoglycemia.

Symptoms of severe hypoglycemia include extreme weakness, blurred vision, sweating, trouble speaking, tremors, stomach pain, confusion, seizure (convulsions), or coma.

Use the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not use extra medicine to make up the missed dose.

It is important to keep HUMULIN 70/30 on hand at all times. Get your prescription refilled before you run out of medicine completely.

Side Effects

Get emergency medical help if you have any of these signs of insulin allergy: itching skin rash over the entire body, wheezing, trouble breathing, fast heart rate, sweating, or feeling like you might pass out.

Call your doctor at once if you have shortness of breath, swelling in your hands or feet, or rapid weight gain (especially if you are taking an oral diabetes medication).

Hypoglycemia, or low blood sugar, is the most common side effect of insulin. Symptoms of low blood sugar may include headache, hunger, sweating, pale skin, irritability, dizziness, feeling shaky, or trouble concentrating. Watch for signs of low blood sugar. Carry a piece of non-dietetic hard candy or glucose tablets with you in case you have low blood sugar.

Tell your doctor if you have itching, swelling, redness, or thickening of the skin where you inject HUMULIN 70/30.

This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects.

Interactions with this medication

Do not change the brand of insulin or syringe you are using without first talking to your doctor or pharmacist. Some brands of insulin and syringes are interchangeable, while others are not. Your doctor and/or pharmacist know which brands can be substituted for one another.

Avoid drinking alcohol. Your blood sugar may become dangerously low if you drink alcohol while using HUMULIN 70/30.

Tell your doctor about all medicines you use, and those you start or stop using during your treatment with insulin, especially:

  • aspirin or other salicylates (including Pepto Bismol);
  • birth control pills;
  • a monoamine oxidase inhibitor (MAOI);
  • oral diabetes medications;
  • steroids (prednisone and others);
  • sulfa drugs (Bactrim, SMZ-TMP, and others); or
  • thyroid medicine.

This list is not complete and many other medicines can increase or decrease the effects of insulin on lowering your blood sugar. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Give a list of all your medicines to any healthcare provider who treats you. Not all possible interactions are listed in this medication guide.

This medication is not expected to be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment.

It is not known whether HUMULIN 70/30 passes into breast milk or if it could harm a nursing baby. Tell your doctor if you are breast-feeding a baby.
